I have been very happy with sipgate: sipgate.co.uk.

I have had an account there for maybe 20 years. The "basic" package does
all that I want:https://www.sipgatebasic.co.uk/. The only quibble is
that the voicemail delay cannot be set to more than, as I recall, 30
seconds. I think that is a "basic" restriction.

I have used it with a large variety of devices and softphones. Currently
I use linphone and twinkle on laptops/desktops. And my router supports a
DECTphone connected to sipgate. Calls sip-to-sip are, of course, free.
You only pay for outgoing calls via the landline interface (I forget the proper
terminology for the moment).

I very seldom need to top up my account, but perhaps I don't make a very
large number of outgoing landline calls.

I also have another couple of sip accounts: with linphone, and iptel.
But those don't provide the landline gateway.
